Welcome to the site for the Church Benefits Association (CBA) Annual Statistical Survey.  Please complete as much information as possible and submit your data by September 1, 2004.  Thank you in advance for your time and participation in this program.

It is structured so that you can fill out both the Pension & Health plan sections multiple times. We'd like to have one filled out for each plan you have available.
